The Preventing Falls Together: A Population Health Tool Kit, is also available on a CD that contains the entire Preventing Falls Together: A Population Health Tool Kit. Ageing Well Together coalitions are active in both Antigonish and Pictou Counties. The coalitions' role is to support seniors to lead healthy, safe and active lives by developing and implementing collaborative community falls prevention and ageing well strategies and by incorporating these strategies into the policies, programs and/or activities of coalition members' own organizations. Member organizations represent a wide spectrum of seniors', caregiver, community, health and social service groups and organizations.

The Pictou County Municipalities Crime Prevention Association provides this free program to all senior residents of Pictou County towns and county which provides important safety awareness & information on a variety of safety topics.

Hospital in the Home, St. Martha's Regional Hospital

Service: Hospital in the Home is an acute home hospitalization service. It enables patients to be treated in the comfort of their homes receiving services which are generally offered in the hospital setting including medical supervision, professional nursing visits, laboratory services and required medical supports/supplies.

Positive Aging Directory is a comprehensive information directory published each year by the Department of Seniors to give Nova Scotia's seniors quick and easy access to the many programs and services for seniors in our province. It also serves as an essential information source for those who work with seniors, many of whom work in partnership with the Department to ensure listings in the directory are accurate and up-to-date.

Service: The VON provides a variety of services 24 hours per day, 7 days per week to all age groups. Services offered are based on individual and community needs, and include home visiting/nursing care, seniors home check-in, dial-A-Ride transportation assistance and the Meals on Wheels/Frozen Food program.
